If you were banned for intentionally feeding, then it would display games in your client when you log in. Please post these linked games and a screenshot of what you get when you log in.
Also, Riot hasn't used "ban codes" since the early part of this decade.

edit 2 - first death was against camille and he could've won - was it a stupid decision, sure, but it's not like it proves intentional. He then dies to minions after his second fight with the jungler there. He then goes back in, stands near a wall, gets deleted by the camille. He then leaves base, immediately backs while not even to inhibitor, then AFKs intentionally the rest of the game.
For a bronze player, I may be able to look past the deaths. The deaths all individually appear slightly suspicious especially for your rank, and together raise quite a bit of suspicion. The lack of any ward placement as well. We can't see in game chat, but how much does anyone want to bet that there was some flaming going on of the jungler for "being late" and getting OP killed?

The standard is quite high - there must be certainty beyond a reasonable doubt that the behavior occurred. Given the replay, which I've just completed watching again (well, the few minutes you were active in, at least), I have determined that beyond a reasonable doubt you intentionally AFKed the game - which is a trolling behavior punishable with an account suspension. Furthermore, I have determined beyond a reasonable doubt that, while none of your three deaths may themselves have been intentional, that you took specific action to spite other players and in taking such action the result was your death - which is griefing in that you, be it willfully or "in the heat of the moment" intentionally ran back to lane and did something so incredibly stupid that no player at any skill level above complete noob should've done (run straight next to a wall into a Camille).
So no, I didn't say that. And just to make it clear, I'm saying the opposite. This punishment is clearly valid and correct.
