This seems to happen quite often. Someone will add me after a game, call me a bunch of racial slurs and block me. I assume they do this so that riot doesn't see how they're racist and are trying to dodge penalties for being toxic.
I managed to grab a screenshot of this last one, since its happening so often I was ready this time.
What can be done? How can I report this?

take that screen shot and write a ticket into support. they are really good at looking into things if you send it to them like that
"What can be done?" You don't add them. If it bothers you, don't take requests after bad games and I've only heard that private chat is private, and is between you and him.
Support ticket, and if you want to follow up yourself do what I do, find the guy on op.gg and bookmark him. About once a week or so I check the folder and see that most (not all) of the people i do this to either stop playing (account suspention) or stop trolling (based on winrate and kda). Many people have a one-off experence and don't see the wrongs that are righted, but if you care enough this tip does help.
The issue is the concept that "Justice must not only be done; it must be seen to be done" should be applied here but without work we cant see it, Riot privacy policys et all.
Censor the name.
