As for the Summoner's Code, there's a link straight to it in the Code of Conduct section of the ToU, but otherwise, you can find it by navigating the site; in the Explore League dropdown at the top left of the webpage, click "Game (Learn about League)", and then from there, mouse over the "Get Started" dropdown, and at the bottom is the Summoner's Code.

I don't know exactly the situation you're running into, but I can at least hazard a guess; no Instant Feedback Report.
Basically, the Instant Feedback Report only occurs when the match you reported a player in results in their account receiving a punishment. However, not receiving an Instant Feedback Report doesn't mean that your report didn't do anything; rather, that it didn't immediately result in a main tier of punishment.
When you validly report another player, the Instant Feedback System logs the game and counts it as an example of poor behavior for their behavioral history, and slows/negates Honor progresson for that player for that match. If the player's behavior is egregious enough, or otherwise consistently poor, the IFS will punish them with a mainline punishment tier (typically a 10-game chat restriction for the first punishment) and drop their Honor Level to 1.
