Officially it isn't known by anyone but Riot. Sometimes you get it, sometimes you do not. Doesn't mean any action was not taken, just that the system did not give you feed back.

Unofficially my friends and I notice reports seem to have more weight when done by multiple players not in a premade, and by players who rarely use the report function. When a player uses a slur and I report on an account I rarely report/play on I am very likely to get instant feedback. When I do so on my main where I lowkey report players every game I never get player feedback even if they used a hard R,F, or N. Last player feedback I saw on my main was in 2017.

If the game you reported the person shows up in their reform card, you'll get a punishment notification.

Keep in mind the reform card only shows players 1-5 recent games as examples of unacceptable behavior, if a game passes the "recent game time frame", it won't show in the reform card .

I would say the multiple non-premade corelation is more due to the offense being higher visibility and more likely to be a more extreme offense. It crosses more lines.

As far as the inconsistancy of the notifications... you can blame trophy hunters for that. A while ago there were players who were intentionally baiting players into bannable chat offenses for the sole purpose of getting them punished, posting screenshots of the notice, and shaming the banned players on other platforms.
