depends, was it intentional?
here's what you do when you suspect someone of intentionally feeding. you don't say shit in chat. you assume the best and try to help them out. if they do better, great! they're not feeding. if they don't, report them on the end screen. again, not saying anything in chat.
if they have a habit of dying a lot, the feedback system will ban them.
If they intentionally went 0/6, yes.
If they unintentionally went 0/6, no.
With a score like that in that time-frame, I would certainly be much more suspicious.
