I mean, the score was 0/17/2 in a 25-min game and judging by the timeline you were back-to-back running up to top lane to die as soon as you respawned. I'll check the replay in a bit but it doesn't look great.
You can attempt to argue your case with support through a ticket.

Replay watched edit: On watching the replay I would say a giant "maybe". Several times during the early-mid game you went in on Jayce despite him having both a significant item and level advantage. Besides that I didn't see anything substantial. I can certainly see why the system would have triggered on this game regardless of if it were ultimately intentional or not. Talk to support, argue your case.
Majority of deaths on your side of the map and you actually out damaged the jg so there's that. Probs a false positive again on riot's part.
Seems like a bad game to me. Singed's playstyle involves dying a lot, but not intentionally; just a product of the playstyle.
I also looked at your match history. looked at where you died etc,. seems like a bad game while also playing singed.
