And yet when I go to the support site, this is the only game listed, the only log that it gives me.

Where are the other games? Where is the "You were reported X times in Y games."? It only lists this one game, and doesn't tell me anything about how often and by how many people I was reported, so I'm forced to assume it was one game with a maximum of 4 reports.

There's nothing zero-tolerance in here, unless you choose to count "%%%%%%%%%%" : / (oh that got censored, it starts with a C and rhymes with lock trucker.)

Are there more logs? I asked to see all of them, the metrics of the number of times I've been reported over how many games. Since that's what I asked for, and this is what I got, I'm forced to assume I was only reported 4 times in a single game at maximum. Could someone please explain to me how that is "being difficult to play with over a large number of matches and teammates"? 1 is not a large number of matches. 4 is not a large number of teammates.

Something else to note is that this was a normal game. I was testing out a new rune page and build on Kat, and I got dumpstered. There was nothing really on the line, yet people far above my Elo (gold / plat / diamond borders across my whole team, except me, silver 1) decided to gang up on me for making mistakes against a Diamond Yasuo main. As Kat. That is not a good matchup and I know it.

Who said that? Mind telling me who told you that because I doubt it was a Rioter, the amount of reports in a single game doesn’t matter, it is the frequency of reports that do.

This is untrue. ONE report in ONE game is enough if the behavior is problematic enough, and this game is indeed enough to have earned a punishment. It's typically true that it takes multiple reports over multiple games to be banned, but each punishment level can trigger off of a single game -- or off of multiple -- depending on your behavior.

because it was THE GAME that did it as of recently probably. Other reports just add to it.

The reform card only shows you 1-5 recent games as examples of unacceptable behavior.

If you want the "you were reported X times in the past Y games", then tier 1 support can give you information for the most recent 20 games.

If you want a longer history, you need to request for someone on the behavioral team, and it is entirely up to them whether they want to give you that information.
