Not talking zero tolerance slang, but sometimes I fail /r to my friend, and end up typing something like "You fucking suck nerd" in chat or other kinds of banter. On a side note, can we get banned if we accidentally fail a /r with a zero tolerance word?
Most likely, yes. Will Riot consider potentially reverse a mistake like that? Maybe, but proving it was meant to be a whisper might be hard.

Just use voice chat, they won't monitor that. If they did, it would be creepy.
well, right now about 99,99% of punishements that are dealt for chat are done by an automatic system. i dont think it knows the difference between a fail whisper and an actual insult
also... EVEN IF it would know the difference, most likely people would abuse it. calling someone something bad with a "fail whisper" as excuse
