Insta-ban behavior?
I was called very homophobic terms and told to go kill myself by another player. Why did they not get banned? Those should be automatic insta-bans.

To answer your question IFS does take some time to go through a game, pretty sure homophobic slurs and telling someone to kill themselves is zero-tolerance. If you don't get an IFS feedback assume the system failed you
Assuming they were reported, those are typically zero tolerance and result in an escalated 14 day punishment. It is possible to get into another game before the report is reviewed. Finally, you are not guaranteed an instand feedback report if the person was punished.
