Disqualified from season rewards?

I saw a page that discussed the season rewards, they seemed nice but it did not seem to explain who would not be eligible for them.

What is the cut-off date for those that have been punished? keep in mind I have been able to use hextech crafting all season with no interruption despite being (in my opinion) incorrectly permabanned and then having that permaban overturned.
