As others have said, it's unlikely that something as small as a name change would help with your toxicity problems, though, if I had to suggest anything, I'd go with a middle-ground of your preference and Hotarµ's suggestion; perhaps something like Tilted Sarcophagus?

As far as tips on how to reform go, though, that's a difficult thing to tackle. While pretty much everyone and their mother can offer tips to vent stress and mitigate the effects of anger, the only real tips I can give regarding reform are these;

Understand your problem. It takes a fair bit of introspection, but knowing exactly what your problem is - down to the roots - can make the world of difference in your ability to reform. If your issues are general toxicity (flaming, negativity) and AFK'ing, then try to think about why you do it. Not just what triggers the response, but why it does, and why, out of all the approaches you could take, you choose that in particular.

Commit to self-betterment. Regardless of if your goal is a small step towards overall reform, or if it's as great and idealistic as changing your behavior altogether, the only way you can actually change yourself is if you want to change and commit to it. Understanding the problem may make a world of difference, but the reform process can stop before it's even started if you don't have the desire and conviction to follow through.

Try to break your reform down into smaller, more achievable goals. Sometimes the goals will already be on the smaller scale (such as with AFK'ing; all you need to do is stick around and play the game out), behavioral change can require a lot of effort, and a lot of breaking-down of the issues that you need to tackle. Not responding to/muting agitators, practicing venting techniques to de-tilt, and limiting your playtime are all viable and valid sub-goals to the overarching goal of reducing toxicity.
I know that these tips are pretty general, but sadly, as mentioned before, that's one of the issues with personal change. It has to be voluntary, and it's not something that one can easily make tips for. Regardless, I do hope that they help you, and if you'd like any further information or advice, don't hesitate to ask.
