Overstepping boundaries? (Poll)

Hi,

So I just finished a game where a player on the enemy team said "You guys should be in France the next time ISIS attacks so you can die".

This is offensive, ignorant, immature, and hurtful to many people. Yes he was reported, but I guess my question is, how does someone like this slip through the cracks? I mean he was abusive/toxic for the entire game, and I can't imagine this was the only game he has ever lashed out. If someone oversteps a line like this, isn't that perma-ban worthy on the spot? IMO it is because THAT is just plain wrong.
