Punishment works on a ladder system

Each punishment has the potential to send you up the ladder to the next tier. You will slowly move back down the ladder over time so long as your behavior isn't poor. Extremely poor behavior or zero tolerance insults/slurs/phrases might cause you to skip steps.
Trolling is punished by a 14 day ban on first offense and perma-ban second offense.
When you are reported your behavior for the game is scanned by an automatic system. If it finds nothing, nothing will happen. Additional reports past the first report mean nothing to the system.
Manual reviews happen, albeit rarely, though it's untold exactly what will draw the attention of an employee. I suspect being reported several times more often than the standard player while simultaneously being cleared by the automatic system is likely to throw a flag up on the account for a review.
Performing negative behaviors and other general rulebreaks can push you towards a punishment - if you've been reported for enough negative behavior (whether it be arguing, passive-aggressive behavior, flaming, etc.), you'll be punished and moved up to the next punishment tier. The punishment tiers are as follows;
If your behavior is especially egregious and falls under Riot's zero-tolerance policy (hate speech, encouragements of self-harm, etc.), your punishment will be escalated appropriately, typically to the 14-day ban.
Additionally, gameplay-related offenses (griefing, trolling, intentionally feeding, otherwise assisting the enemy team) will result in a 14-day suspension for the first offense when verified.
When you get punished, your Honor is stripped and locked - if I'm not mistaken, the first offense typically only results in dropping to Honor Level 1, but offenses after that without reform (or egregious offenses, but don't quote me on that one) will drop you to Dishonorable (Honor Level 0).
Punishment tiers can de-escalate if you've proven sufficient reform, and played enough games without valid reports being made against you. It takes quite a long while to de-escalate a tier, though - I've heard it averages around a couple of months of consistent play in order to go down a tier.
Also, the IFS only cares that the number of reports made against a player is greater than zero - if there are anywhere from 1-9 reports made against the player, it's counted as a single report, and the player is reviewed. If there are no reports made, the IFS does not review the player's behavior, and consequently, will not apply a punishment, even if one would be warranted.
And, lastly; Valid reports, even if they do not lead to a punishment in and of themselves, slow or otherwise hamper a player's Honor progress. It may not result in the punishment and honor-stripping outright, but it does count towards it.
