Tribunal was the old system where people decided whether a player was intentionally being toxic now we have instant feed back where it makes the decision automatically.
Also you have to be reported for multiple games I believe.

After extended periods of neutral/positive behavior, your account will gradually drop down in punishment tiers. Valid reports will agitate and slow the process (or escalate the punishment)*.
Tribunal was a system that involved the community reviewing cases to determine the validity of reports. This has been down for maintenance for awhile. The current system is heavily automated, yes. Rioters do still review cases themselves though, more often the ones that lead to suspension, permanent bans, or extreme cases. If you feel you are ever falsely punished by the automated system, you can send a support ticket and an actual Rioter will investigate your case (this may or may not work against you). If you would like a community opinion on whether or not what you did deserved punishment, feel free to post the supplied chat logs that come with the reform card (in client and to your email) and fellows players will evaluate it.
False reports are voided and will not count against you, regardless of the number of reports. To use math for example, if 5 people falsely reported you, it would like adding 0 to your current flag count five times. The result is unchanged.
