Do perma-banned accounts ever come under review? If so; how often does that process occur?

Automatically, no not really. If there was a (bot/cheating) ban wave that hit some falsely flagged accounts they'd go back and unban those, but otherwise there is no automatic review to remove permabans. They can manually review a case if you request it, but unless it was a completely unfair or false ban it won't be lifted.
