How to improve the "Help us Improve" Feedback

Hi there Riot,

this is an open letter, after filling out this Feedback for the 10th time... i was wondering why it stayed (Feedback) the same way it did in all those years.

I would have a huge improvement in mind to get more participation, and a better way of understanding what part/bracket of the ranked community is filling this out. This improvement is focused for me entirely because of the question "what level is your champion/account)" i mean only 1-30, but what about "elo" or rankings? like you already ask the question, if i play ranked .... so why not Example Question:

Question: In what devision are you? Bronze/Silver/Gold etc.

or even better Connect the feedback to the accounts, i know that this would destroy anonymity on which feedbacks are connected to. But giving those who fill this Feedback an RP/IP award (or summoner icon, ward skins, champions skins, etc.) would probably encourage them to fill this forms out. And give your Analytics team a great view of what really is important - like what is more important that 20,343,120 people get more champion storyline or history with a skin (connected to that story) or just some random new champion that only 1,023,420 people want?... then seeing in which bracket they are there would play a huge decision making on what is the best strategy to approach this kinds of things...
