What just happened? Hacking?

This may sound odd but i was recently in skype with my duo and suddenly they were logged out of league during a ranked game and i found this peculiar. She had to restart the computer and as she was logging in, it said "invalid username/pw combination" So she went to go change her pw and even then it still said invalid, so im slightly confused as to what happened and why we lost 16 lp
