Fiora (not a complaint)

Has anyone else been experiencing how her Q (lunge) sometimes doesn't proc vitals when it most likely should of? Also her ult sometimes doesn't… go off? (For lack of a better word) I'm pretty sure there are some other ults that do the same thing, probably something to do with a movement command right after.
