Create a Monstrosity of a Champ/Item II

I made a thread with the same title before and it received a lot of replies with hilarious/broken champion fusions. I wanted to bring it back with a twist of adding a summoner spell fusion as well. I'd like to see what crazy things people can come up with! Go wild!

If making a Champ fusion use the following template. One champion per slot. Example:

Pokey Support (aka YOU WANTED TO FARM?! HECK NO!) Base:
Passive: Q:
W: E: R:

For a Summoner Spell fusion simply combine two together. Example:

+ I would love to have this spell in-game. It would change the gameplay of bottom lane so much!
