Worst mistake in LoL...
I want to hear about a time where you seriously fucked up in League.
Mine would be trying to 1v5 as

Leaving Warwick unbanned in ranked because you thought the other team wouldn't pick him first.
I has made many mistakes, but most are files under "every time I play mid".
Almost always a Baron call when slightly ahead. I think this is the worst mistake for almost every player. A bad baron call can turn a won game in to a lost game. The throws are real.
Taking the baron debuffs so the other team can walk in, ace you, then get a 20% health Baron for themselves, then take an inhibitor while you're aced before recalling for the final push. It's probably even worse with the new Baron, fortunately I haven't had it happen yet (had a teammate make a terrible baron call recently, but I survived, warded, then stole Baron successfully afterwards).
Accidentally bodyblocking our Sona from escaping the jungle, which led to her being grabbed by a hook, the rest of us surviving but with Sona down, we completely lost the fight for our inhib.
Then again, she WAS pretty toxic and nasty to her ADC…

Basically if your team had 30 kills, and Darius had 25+, Your playing him wrong.

Probably when I made it to silver 2 in provisionals (which I thought was pretty good since I haven't been playing that long) and my friend and I continued to play tired until we dropped all the way to silver 5. I kinda compare it to when you're at a roulette table and you're on a losing streak, so you decide to double your bet in the hopes to make it back even, then you lose again, double, lose again until you realise you're $5,000,000 in debt and your kneecaps are getting smashed by a thug that goes by the name of Meatfist or something.

accidentally throwing the game by telling my team i want to wait for full build (was 1000 gold away from my last item) instead of just taking out the nexus turrets after 1v5ing the enemy team at baron resulting in a quadra and me stealing baron without ever attacking it (
op) and the fed riven barely escaping while i had 75% hp left as AD

i q'd the enemy jungler when they were doing dragon and i saw the enemy was doing dragon i pressed q a second time and died
but in the end i stole the dragon by kicking the enemy jungler into the dragon but then my team lost 2 inhib turrets and 1 inhib because i was dead for 30 seconds so i couldn't help the team fights
