So about a year ago when I was new to jungle, and I was quite bad at it, I got pissed off and got flamed at by my team because I failed jungling. That resulted in a 1st offense, however, now I was in another game where I was also a bit tilted and for some reason I got the 3rd offense, even though I have never gotten a 2nd offense. Shouldn't I be getting a 2nd offense? I don't remember getting one at all.
Extreme toxicity like racial slurs, homophobia, etc can cause you to skip punishments. If you are sure that wasn't the case try submitting a support ticket.
