What am I doing wrong?

I need help.
What am i doing wrong in my games. Feels like when I do bad we win and when I do good we lose, what can I do to improve? All help is appreciated.

Playing Teemo top when you have no tank is probably why you lost. They have 3 tanks, and you tried to carry from top lane. Teemo is great for single target but you bring nothing to team fights. You can't tank. Can't peel, yeah you get kills, and probably won top lane against Nasus. But winning lane doesn't mean you win the game. It's a team game and your team needed something specific and you chose something else. Hell you didn't even hinder Nasus's farm.
