When you play Karma and your ADC threatens to report you for not healing her. The sad part is... That one happened in mid-Silver... [zombie-brand-facepalm]
When you galio ult and your teammates flash away from it
When you get rank shamed in ARAM.
I don't remember the stupid stuff people did in ranked. I was too busy getting flamed for being nervous.
When you jungler is spanish
When your team blames you for not dividing yourself into 4 different people to assist every lane and still being able to farm jungle.
When people blame you for not grabbing drake when the the whole enemy team guarding drake.
When top gives first blood and you get blamed for being bot side.

You see someone building armor against Katarina because she's building AD. Someone complains about Nasus chunking with Spirit Fire when he's using a non-AP build. Yasuo complains about pretty much any other champion (we can give him a pass for hating on Zoe).

One of the most obvious tells is when your laners don't watch the jungle buffs and just AFK under their towers for the first 2 minutes of the game like that portion of the game doesn't exist or something.
...you have stacks of Relic Shield but your ADC tells you to stop taking minions or they'll feed.

when your support has relic stacks but they're too blind to see the cannon so you both lose it

Getting asked "why did you just stand there" when you die due to a root/stun. Getting asked "why did you walk towards them" when you got taunted/charmed. Just people not understanding status affects in general.
When you get stuck inside Veiger cage and try not to touch it so you don't get stunned even tho that Veiger is your teammate
Hell even I'm gold and I still do that because of how much I played against Veiger
Actually and when your teammates just run to Veiger's E everytime they see it , hell every single time in bronze they MUST run to veiger's cage for an unknown reason.

Retardos using Jinx ult, not to execute out of range low healths. But in full on 1v1 fights. Also idiots who think stepping out of Kindred ult will help them.
