I would just like to say before anyone gets into it. No! Irelia and Sivir are NOT sisters, since just like Jinx and Kai' Sa's interaction, it's simply used as a term, a gesture.

Considering that Sivir is Shuriman and Irelia are Ionian, it would be completely impossible for them to be related.
