Running out of mana vs champ that uses no mana
Fatal misclicks, having my fingers on the wrong keys, running out of mana right before the final blow, them escaping with 1 health, etc.
When I or a teammate dies or an enemy gets away simply because someone wanted to farm a lane rather than listen to some pings and come down river to head them off.

Btw I did get the inhib and we proceeded to win on the next push. Guess it's people who don't understand objectives that gets me.
