ELO HELL

Hi i am a long time league player, I started my account up in 2012 and I have a problem with ranking. I have played game apron game and even tho I do everything in my power I keep on loosing. If I do not have a bad early game I am usually strong in late, but in late game i always loose. In late game I try to instruct my teammates so that we can come out beneficial in late game. Lately I have had a problem with team fights because my team scatter or every one focuses me. My intention was to ask YOU, whoever reading this help. I need a solid team to run with on ranked. I have 1 lp in bronze 5 and I am desperate, pls help.
