Holy shit, It's not about ANY OF THOSE. It's about HOW WELL YOU PLAY YOUR CHAMPION COMPARED TO EVERYONE ELSE WHO PLAYS THAT CHAMPION IN THAT ROLE.
You want an S+? Play something like ezreal Jungle and do crazy well. Do something like AP soraka Top and do amazingly well.
Otherwise, you have to literally PLAY BETTER THAN OTHER PEOPLE WHO PLAY THAT CHAMP.
Unfortunately, Riot has stated that they will not release info on what will get you an S due to fears of people gaming the system rather than trying to win (e.g. not taking the nexus in order to get more CS or drop more wards)/
Idk what to tell you other than "play well." They intentionally don't tell you what it actually measures to avoid abuse cases, so your only bet is to, as we say in in the old country; git gud
It is indeed based on ones percentile performance compared to all other players playing that particular champ in that particular role. If the community on the whole tends to produce shitty performances, then one can do fairly bad in absolute terms but still get a good grade, and similarly if the community on the whole has a lot of top tier performances on a champ/role combo than even a godlike performance in absolute terms can lead to 'only' an A instead of an S.
It does seem surprisingly that that performance by diana warranted an S, but I guess the masses are bad at diana mid?
From what ive seen, It is almost always based on kills. Ive supported my a** off and and had the best support kill death ratio. nope, didnt kill enough aparently. I would have people in our game going roughly 20-11-9 with an assasin and would beat my 9-4-28 garanteed.
playing an assasin that cant be clicked on most of the time seems the way you get an easy S. Making sure your team gets kills without killing yourself dosnt.
You are graded against other Volibear, other Diana etc in the same role.
If every support Diana goes 0/10/0 and you go 2/8/1, you are playing better than the average and therefore will get a better grade.
That's why getting S as an assassin on Definitely not Dominion is difficult, every assassin goes 20/5/20 every game, and you have to do much better than that to get it.
