I cant rank up

I have been trying for so long to get out of bronze (this years placement did not help) but I just can't manage it. I tried contacting people on boards before and it somewhat helped as now I decided to focus on 2 champ per lane.

I play (Top: Sion, Irelia and mid(can also play top) Akali and Talon. I know I am weaker mid but I still do quite ok. With Sion, I become a mega tank and often get the most assist on the team. With Irelia I very often get fed and help my team win (almost never feeding). I am decent but newer with Talon and Akali.

Some of the flaws I know I have are: I'm not the best at csing (but working on it) I sometimes get cocky and pick stupid fights. I get stressed in close games and I mess up my combos (mostly applies to Akali) I don't place wards enough / I forget to place them.

I have taken a big break from ranked and have come back to it a couple months ago because of my experience. I would easily reach promos then lose the promos (often because of uncontrollable situation afks or feeders.) This happened to me at least 6 times and one time I made it but lost the game after that and went back down.

What I am asking is for someone to help me at least get to silver by coaching me by looking at my games and telling me what I could do to make it there. I am looking for someone that is above silver.
