Need help/tips for ranked climb

Above is my OP.gg. I used to be plat like 5 or 4 in season 6 and was G5 last season simply because I didn't play ranked and just kept learning the game in normals along with some friends. Most of those normals consisted usually of a mix between silver-plat players. I have trouble understanding why I'm losing these games or winning them so damn inconsistently. I make mistakes but I do try to fix them and still feel that when I fix one another big mistake or barrier gets in the way of me doing better. Any tips or genuine explanations as to what might be happening is appreciated. At this point I've taken small breaks in between to refresh myself but nothing feels like it's getting better.
