Make the damage and heal scale off of missing health to make it strongest at low HP, but comes with the tradeoff of making you vulnerable to burst

Or, remove the damage, and make the heal stack up each time you proc it in a reasonable time frame, for higher sustain but requiring a longer period of time to become fully effective

Both would work well to make you feel undying, I think
