Out of Range Monitor

Hello. So I've been using a new graphics card and I keep getting this thing in game where my monitor screen will go black and then say "Out Of Range" The card is a Radeon 4850 and I had tried some other cards before such as 2 different Radeon 4350's and a NVIDIA 8400 GS. All of them will glitch out like this and there is no way to get back in except sometimes Alt+Enter works. Usually it happens at the start of the game so my team remakes and I get the penalty. My cpu is a Core 2 Duo and I know these specs are quite old but I had also been running with the integrated chipset before the graphics cards without this problem being so persistent. I've switched to the borderless mode for ~2 days and it hasn't happened in game but during champ select. I really don't want to go back to the 20 fps integrated chipset :(
