Why I dodge a ranked game (answer to ask Riot)

In the last Ask Riot (http://nexus.leagueoflegends.com/2017/02/ask-riot-where-is-butchers-bridge/) they mentioned a feedback system to question why we dodge games. Well, I am sure we all have reasons, but here is the most frequent reason I dodge a game:

When a teammate ban the champion I declared I was going to play.

I also offer my teammates to dodge FOR them if I accidentally banned their declared champion (because I was too focus on something else and didn't notice what they declared), because I know how it can be frustrating.

A simple warning system could fix this issue... which happens quite often, and usually not on purpose.

Why do you guys dodge games?
