@meddler If your biggest problem is Vlad's Q healing in lane, why not move the healing passive?

You could remove the flat heal and add the ratio to W, having it scale with rank. It would give a little more incentive to level W, as well as effectively remove any significant sustain from laning.

In order to allow him to function better in the meta, add a decaying MS buff on the Q return particle to help alleviate the casting delay on his Q and E that he's continually spamming. Not to add mobility, but to make him more usable.
