Question regarding Titanic Hydra interaction

I know tiamat has an internal cooldown, so things that apply on-hit effects more than once don't trigger it more than once. Will this apply for the on hit bonus max HP damage, or just the AOE?

Basically, I want to know how it interacts with renektons w (furious or otherwise) and shyvana's q.
