Where is the quality control?

I'm basically echoing everyone elses thought today about:

1. Bring back Team Builder
2. Why are solo queue and party queue put in the same game?

But my thread here is basically to ask: Why the hell didn't anyone outside of the ivory tower focus group this. I swear this only happens in Tech and Finance where people just think "well there's 12 of us making the decisions here, and we're brilliant so we don't need to ask anyone else."

If the X number of people in the room, making those decisions just called a relative and ran the concept by them, they could have avoided this train wreck. Is it me, or do you not need to be especially intelligent to see how this will end?
