How to implement fair dodging in champ select for any queue!

Have a dodge button in the champ select, when you press it it will make a pop-out appear similar to when you report someone. It will look like this:

You are about to dodge the game! Please explain why...

Player dodging: <Username of player> Reason: <Drop down box with options listed below (Toxic, trolling, and dodging at own risk)> Player accused (this option only appears in certain cases): <Select a player on your team>

The reasons available should be in two categories: First one where the "Player accused" option appears, in this category are two options: "Trolling teammate" (Don't report off-meta champs, unless it's an obvious troll.) Also the second option is "Toxic teammate" (This one is obvious, things like 'mid or afk' etc.)

The second category has one option: "Dodge at your own risk" This is the same as the current dodging system. You lose LP if in ranked, counts as a loss, get the timer. Same stuff we know currently.
