This has happened more than once:
I look at my selected rune page in champ select and make sure it says the proper page is selected, but upon loading into the game a completely different page I have NEVER used before loads in.
It's cost me 2 games so far having completely wrong runes (IE open spellbook and resolve on sona)
Yes! I don't know what Riot is doing, but it happened to me 4 times so far...just lost a ranked because of getting tank runes on katarina...plus we had an afk, but that doesn't matter now. I had the normal dmg runes set in from previous matches, didn't even touch it in champ select and it said that I have what I wanted, but in the loading screen I saw something completely different and in-game I realized that the game gave me a magic resist tank page against a full AD enemy team...that seems fair...

YES! It has given me my klepto page like 4 times when I'm sure its not the page i took.
Whenever I login it it defaults to APC page which is my second mastery page, I always play ADC my top mastery page. Was hoping it could default to use my top ADC mastery on login instead.
