Issues with Spell Order

Questions:

1. why is it that on certain occasions i am able to hit my flash key, hear the sound of flash going off but still end up dead with no used flash? As if i had beat the other player in moves but yet i didnt? please explain

2. Again with flash, and other dashes tele etc, i am able to use a movement abil and at times i am cc'd at the other end of my move. Again, something needs to change, either i make it and take no cc or i dont and remain in place. not both
