I'd have to say
Toxicity corrupts and degrades the skill and enjoyment of all players. This leads to disconnections and griefing. Therefore toxicity is the worse in my opinion.
Myself. Sometimes I get impatient and stop thinking clearly about what needs to be done in order to win the game. This causes me to feel my team mates are unskilled or I wasn't able to hit an activatable item in time due to lag. But in the end, it's me that's the problem.
There needs to be a 'Me, myself and I' option.
Because as much as I hate to admit it, I'm my own biggest issue. I refuse to admit I'm going on tilt and it will end up costing me. Trolls are pretty rare, I can mute toxic people, afkers aren't that common, and my lag is manageable. Even the skillgap isn't that wide.
Where is the Rito option?
Bad matchmaking by far. After that comes abusive and unfun champion design and balancing and then wwaaaaaayy down the list; everything else.
I'd say number one is that one guy on your team that always gets caught, but that's not an option.
I'd say number two is anyone that suggests doing Baron. 95% of the time the team that starts it gets rolled because it does decent damage now. It's always been a throw magnet, but it feels like that's ALL it is now. If you can take Baron safely you've got such an advantage you could take some towers or inhibs instead, and they'd do more for you.
When the game is a lost 4v5 or rarely even rarely a 3v5 but your team still thinks you can win forcing you to play and waste another 20 min of your life after the first surrender vote.
Toxic people and Ragers.
Makes games horrible for EVERYONE.
I don't really care about DCs or feeders, cause that's inevitable, and sometimes, the person isn't at fault doing it. TOXIC PEOPLE however, can be reduced in numbers and CAN BE PREVENTED.
SKill cap difference? As long it doesn't cause toxicity, its ok to learn from the pros once in a while.
It's pretty damn close between bad matchmaking, toxic teammates, and afks.

The thing about a toxic player in a match is that they usually make all of the other game ruining experiences in the poll (Excluding lag) happen more frequently. The toxic player tends to grief your team and/or DC. Sometimes they are toxic because of poor matchmaking, but sometimes it is just how they are.
I think you need more options. Often, despite a severely losing lane in a game (any lane, not specifically mine and not always not mine) there are plenty of opportunities to come back or win. Unfortunately, in most of the games I play I believe the biggest reason for losing by far is lack of teamwork/opportunity. Teammates will not split push, focus objectives, ward, or group as necessary and may roam repeatedly feeding.
If you are losing really bad force split push if your team allows it but don't do it with zero vision and know when to back off. Keep the pressure may let your team bide time to catch up and this often is a great solution unless they can all in down a given lane when 1-2 of you aren't there to defend a enemy team push. The one time this doesn't work at all is when they can wipe your entire team and turret dive without issue, not exactly common but the enemy can become so grossly over fed or have the right champs/comp for this opportunity.
Properly focus, don't just attack the lowest HP person and zone targets in a fight. Completely locking down the 1-2 core dmg dealers even if it means taking in some damage from the tank/bruiser may be completely worth it as it caps their enemy team dmg by possibly 50-75+%, especially if they are fed.
See the enemy blow 2-3 CC and maybe several misfires/misses going for a target who dodged it or most of it don't back off, immediately engage and take advantage of a completely uneven fight even if they are fed! Don't play poke the enemy for 5 dmg and back off until they enemy has forced you into your base from their jungle because you refuse to find or make opportunities and certainly don't wait until enemy Blitz finally lands a good grab on your team. Engage on YOUR team's terms!
If your DPS or AP is so grossly behind due to getting slammed in lane do what it takes to bide time and keep moderate or as much map control as possible and defend until they can at least halfway farm up enough so that they will be able to contribute something in fights. From there they may make up the money they need and eventually completely catch up (this is particularly true for DPS champs and snowball champs like Katarina/Akali).
Jungler should help the lanes that can benefit from it the most and not repeatedly ganking the losing lane or the slightly behind lane. They can defend it while you shop sometimes but for the most part if you are losing don't keep wandering into middle of lane to die, sacrifice some CS and just last hit as much as possible even at tower. All too often I see a jungler helping lanes it shouldn't someone repeatedly feeding 0/6/x because they keep trying to duel in lane, just give it up if you can't win it and DON'T even TRY to harass them at all just focus entirely on taking minimal harass, safe positioning, and last hitting as much as possible. This will almost always work as not many champs can simply dive and farm you under tower if you keep pot up (pots are important!). There are a few cases where you will just get gutted under your tower (Syndra, Zed, Lee Sin, Katarina if fed, Akali, Irelia, Renekton, etc.) but most often the enemy can't kill you if you keep properly topped up with potions and avoid taking too much dmg regardless if they are fed several kills or not. I also see all too often people refuse to swap lanes until they are 3+ deaths in because of their pride or whatever reason but sometimes swapping can be really advantageous and even doubly so if its a good swap for both teammates (the armor you have or whatever doesn't matter half as much now, Chalice, etc.). You see lane swapping a ton in higher level play and for good reason, beyond just the advantage benefits (adc/sup vs solo lane).
Honestly, I see the other choices about evenly and not very often as the cause with lag/ping being very rare case (usually Riot's fault causing fairly even issues across the board if so). Verbal abuse might be slightly higher then the rest of the options and can impact the match in many ways.
tl;dr Poor teamwork/opportunist is biggest cause of a loss, imo.

This poll isn't worth jack if it doesn't have an option for broken/overpowered champions
I strictly play Dominion now. For the main reason of leaving is the humongous skill gap when it comes to playing the game. It has legit nothing to do with champions or trolls. It's legitimately the inept skill players carry with them when joining a game.

the ragers IMO, it has gotten to the point where i truly hate playing this game. the best advice i can give to a person who wants to start playing this game is.... play with friends because the community is sh'it. the one reason i still play, is because i put so much money into this game and i hate walking away from an investment.
i remember when the punishment for toxic players was: warning, 1 day ban, 1 week ban, 2 week ban, then perma.
now its: warning, chat ban, chat ban for longer, chat ban for even longer, kicked out of rank and pushed into normal games, 1 day ban, 1 week ban,2 week ban, then perma.
why is riot now starting to baby toxic players? when i buy RP that means am supporting the game, why cant you support me and do something about your community.
You missed the bad balancing option in the poll?? balance is a massive issue

feeder(not someone who lost lane, or someone that is having a bad game, someone that goes 0-10 first 7 minutes of game and u are wondering how someone who is lvl 30 did not know when to stop going at the enemy.
afks/dcs (people who do not even connect in the first place or people who dissapear 10 minutes into the game are pretty much GG unless those special games where u can 4 v 5).
toxicity(ragers who talk about teammates mistakes, people arguing,people trolling).
I'd say it's 50/50 between huge ping spikes/DCs and people autolocking champions before everyone is even in the lobby.
