I have big lag spikes since last patch. Any help?

Basicly the title. Sometimes I can have normal ms whole game, sometimes I play on 200ms whole match and sometimes I have normal 43ms with spikes to 600+ms. It didn't happen before, started since last patch. Can I get some help please?
