Mace of Spades only gets 1/3 of usual spell vamp/true heal and reduced time slow from Rylai's while it can only hit one at a time. Is it because it can hit multiple targets in succession like chain abilities or just because of the damage it can do?

Probably the former. Although Rylai's, if you look closely, has a multi-hit portion to the passive. As for spellvamp, likely due to how it hits multiple times in one cast. Otherwise, you would be getting three full spellvamps off one spell, which wouldn't work well for balance.
It's not. If I had to guess I'd say it's treated as a DOT spell for all slow and heal purposes, simply because it's a three proc spell.
