My games are looking like this more often than not, and I'm wondering if anyone else is seeing similar lag issues?
If not, how can I tell if it's me or my ISP?

Notice, you don't have much lag, what you have is a serious packet-loss problem. My recent games don't look anything like this. My 5 games yesterday all have zero packet loss.
Are you on wifi, or a wired connection?
I recommend getting PingPlotterPro (free). You can run it and try to see where the packet loss happens in the path from you to riot.
