I don't understand how teams at worlds are still so bad at the pick and ban phase

If you have first pick, you should always save your solos for the last rotation to guarantee you get to counter pick one lane. If you're going to differentiate from this, it should be for something that is so OP that you have to first pick (which doesn't really exist) or it should be a flex pick so you still might have counter potential.

And then teams are counter picking themselves like crazy. Yasuo into Darius, Olaf into Kennen, and Fiora into Olaf? wtf
