Ekko Bug With Q When He Hits 45% CDR

Sorry if this thread has already been made. I don't know if it is just me, but I just had a game where I achieved 45% CDR with ekko and his Q was not doing damage the second time he throws it out. Also, I also noticed something else that the 3rd hit of his passive is not effectively activating on hit effects like lichbane which can be a lot of damage loss. I was just curious if anyone else who plays Ekko was experiencing this problem as well.
