Dynamic queue champ select bugged out and got stuck at 0 seconds for one of the bans, then eventually went back to the home screen (not in queue anymore). When I clicked Play I got the 5 minute penalty notice for leaving a queue. Took a screenshot and added link to imgur.com page:
