KDA is not the only thing that determines grades. Nor CS. Here are some things that stop you from getting S+

Pink Wards: You bought/used none.
Enemy Wards: You only destroyed 2.
Dragon/Rift Hearld/Baron: Looks like you killed 1 dragon, did you help get the other 2 and baron?
Items: Did you used an elixir?
