Talisman of Ascension GAME PLAY BUG

It seems that when you reach the 650 gold mark of the talisman of ascension if your inventory is full you will not gain the extra skill point quest reward

I have played a couple games where this has happened. My bad for always having those extra red wards I guess.

Could the reward just automatically grant you the skill point instead of being an item?

It does, and they said it on the patch notes. If your inventory is full it will automatically give you the skill point as long as you're out of combat. I've had it happen several times, it's just hard to notice if you don't hit level 18, just clicking the skill up and going back to playing is a very ingrained habit because no other item does anything like this.
