This happened multiple occasions(including in other patches). Also there's a e bug I think where you can like do a moonwalk when you e and move your mouse. I don't know if that is a bug

I can confirm about the death model, I've encountered this problem very often playing with Slay Belle Katarina too. Actually this is a very very old bug since it happened me like 2 years ago and still doing.
