black screen.
when i start league it was ok so i was about to play rank someone left during the first one got reque it pops and i had it minimized when it open it had a black screen i had to close it and i lost points cuz of that

You don't lose points for not accepting the que. Unless you didn't pick a champ.
I had this happen today as well, though not in a ranked game. I clicked accept to enter champ select, and when it swapped over, all i got was a black screen. I could chat with people on my friend's list and such, but couldn't see anything else. Ended up having to close league and relog. Strangely, it didn't lock me out of the queue like I expected it to do. I did take a screenshot of it.
