Ult Icon not going on Cooldown

Just as the Title says, played a game today as Yasou and at some point during the game it glitched out; I used my ult but the icon never shows it being used so the rest of the match my team had no idea if it was up or not and I had to just remember and guess its timer on my own. Has this been happening to others? It's happened to me before but it seems more frequent this patch.
